• We provide services to facility staff that result in their job is easier to carry out • We handle working with the patient, their family and their physicians • We coordinate care and communication • HOSPICE INSPIRIS provides necessary documentation to help ensure licensure requirements such as Plan of Care. • Our Team is available to supplement patient care. • Consistent care team for each your facility - which enhances continuity of care. • Provides a standard of care based on regulations identified by Medicare. • Medications/ Medical Supplies are delivered right to the door including briefs, chux’s and barriers. • Patients can continue to be treated by their referring Physician or a hospice medical director. • HOSPICE INSPIRIS offers bereavement/memorial services to residents and care home staff. • Provides in-services to your staff regarding a variety of hospice related topics. • Refill of medisets and narcotic medication counts weekly to assist in compliance.
When
is hospice appropriate?
• The person has a life limiting prognosis with six months or less to live. • A significant decline in the person’s physical or mental health status is noticed. • The patient requires frequent visits to the MD’s office or the Emergency Room. • The patient’s level of mobility has declined requiring frequent intervention by caregivers or care home staff • The patient is losing weight which is not intentional. • The patient is becoming weaker and requires more assistance with activities of daily living. • The patient’s disease is progressing at a rapid rate or a new diagnosis has been added. • The patient has recently chosen to stop aggressive treatment options.
