When
is hospice appropriate?
• The person has a life limiting prognosis with six months or less to live. • A significant decline in the person’s physical or mental health status is noticed. • The patient requires frequent visits to the MD’s office or the Emergency Room. • The patient’s level of mobility has declined requiring frequent intervention by caregivers or care home staff • The patient is losing weight which is not intentional. • The patient is becoming weaker and requires more assistance with activities of daily living. • The patient’s disease is progressing at a rapid rate or a new diagnosis has been added. • The patient has recently chosen to stop aggressive treatment options.
