Case Manager / Phoenix, AZ
(Full time and Pool Positions)
The RN Hospice Case Manager works in a team approach with an LPN to coordinate the care for a specific patient case load. Self schedules appointments. Performs hands-on patient assessments, manages symptoms of the disease process, and communicates with a physician for orders. Provides instruction and training to patients, families and care center staff. Supports patient and family as changes occur during the course of the disease process. Updates care plans and medication records. Participates in team meetings with the Medical Director.
Qualifications:
RN from an accredited program and current AZ State RN license in good standing required. One to two years acute care nursing experience or one year in home health, hospice or comparable experience is preferred. Requires compassion and strong assessment, organizational, communication, listening and problem solving skills. Demonstrated aptitude for the hospice philosophy and comfort care is a must. Successful candidate will be a self-starter who enjoys autonomy but is a team player. Bilingual a plus.
