At HOSPICE INSPIRIS, we not only focus on the special needs of your loved one, but also provide support to your family. Our care offers a “team” approach to meet the physical, social, emotional and spiritual needs of those who need care and their family. HOSPICE INSPIRIS recognizes that every person is unique. With the expert guidance and counseling of our Hospice Team, loved ones and their families can make their own decisions about what is important to them. A plan of care is developed in order to meet the unique personal and cultural needs of each patient and their family. The team includes input from your loved one who will receive care, family, doctors, nurses, nurse’s aides, medical social workers, chaplains and volunteers. The Care Team The HOSPICE INSPIRIS team is here to attend to all your hospice needs in a caring and coordinated way. Because your loved one may require support 24 hours a day, seven days a week, we operate as a coordinated small team of caregivers. HOSPICE INSPIRIS provides a responsible individual to be available to help provide and coordinate care.
The Primary Care Physician
Because your loved one’s primary physician is best acquainted with his or her illness, their physician can continue to direct care. We understand and respect this relationship. With a strong emphasis placed on on-going and open communication with the physician, HOSPICE INSPIRIS assures that all of your loved one’s needs are addressed. If your loved one is in need of a primary care physician we can arrange for one that understands hospice care.
The Medical Director
The HOSPICE INSPIRIS Medical Director is responsible for the overall medical supervision of your Hospice Team. The Medical Director acts as a liaison to other physicians in the community and may be called on by the Primary Care Physician to visit the your loved one in the home setting.
The Nurse
Through regularly scheduled visits, the HOSPICE INSPIRIS Nurse addresses medical concerns and orders medications related to the care plan. During their visit, the Hospice nurse will explain the condition of the patient and teach family members how best to provide physical and emotional care.
The Nurses Aide
As part of our Care Team, a Certified Nurses Aide will come to the home setting and offer to assist the patient with personal care needs. This includes assisting the patient with bathing, shampooing, shaving and changing bed linens.
The Social Worker
The HOSPICE INSPIRIS Social Worker is familiar with the many feelings and stressors that arise when someone you love is ill. Through excellent listening skills and deep respect for confidentiality, the HOSPICE INSPIRIS Social Worker assesses the emotional and financial needs of your ill loved one and family and provides the necessary counseling to ease the burden of these concerns. The HOSPICE INSPIRIS Social Worker provides:
- Emotional Support
- Guidance regarding the loss and grief process
- Assisting families in coping and adjusting to the life threatening illness of their loved one
- Helping children cope with a serious illness and death of a loved one
- Assisting families in communicating more openly Assistance with insurance applications, filing claims, and other financial resources
- Guidance with potential placement arrangements
- Assistance with Funeral Planning
- Assistance with Legal issues and Advanced Directives
The Chaplain
HOSPICE INSPIRIS values the diverse spiritual needs of those we care for. HOSPICE INSPIRIS Chaplains provide spiritual support to your loved ones and your family from a variety of denominations and belief support systems. In many situations, families have strong ties to their own Church or Synagogue. With this in mind, The HOSPICE INSPIRIS Chaplain can help arrange for their service in your home environment.
