HOSPICE INSPIRIS has created a program for those that are not quite ready for hospice, but do require some additional care or resources. Through this program we provide support, education and facilitation of community resources to meet the special needs of those with chronic or serious illness and their loved ones. Services can be provided in a home or facility. HOSPICE INSPIRIS may work with a Hospital, Skilled Nursing Facility, Home Health Agency or other community based organization of your loved one’s choice. HOSPICE INSPIRIS recognizes that symptoms are often caused and/or exacerbated by psychosocial, social, spiritual and financial concerns. We believe that palliative care is interdisciplinary care that relieves suffering and improve quality of life.
Your HOSPICE INSPIRIS Palliative Care Team may include:
• Nursing visits that are consultative. Direct care of your loved one is not provided, but your nurse will review medications, answer questions, and collaborate with your loved one’s primary physician. • Social Workers who will help your loved one and their family deal with the emotional stress caused by the illness. The Social Worker can also assist you in decision making, offer options concerning finances and act as a liaison to community agencies and resources. • Chaplains who offer spiritual council, guidance and support regardless of faith. Your chaplain may also help “connect” your loved one with a specific belief system, local spiritual leader or place of worship. • Volunteers may be available to assist the patient or family for short respite, companionship or simple errands.
